Is there a form that I need to complete to request copies of my medical records and x-rays be sent to another doctor?

Yes, the office has a medical records form that has to be completed. Payment is also due at the time of the request. An outside company handles our medical records requests. Once a request is turned in, the company will process the request within 7 to 10 days.

I am looking for an application

I have just graduated with a Nutrition Science/Pre-Med degree. I was wondering if you were looking to hire a file clerk or administration position.

I requested an appt. on line, spoke with office secretary and advised to send copy of MRI, copy was faxed, followup call was to be made week of 8/22/11 or I was to call if that has not happened, called Friday 8/26/11 left message on voice mail for office secretary. No return call, called today 8/29/11, spoke with customer service representative, shared problem, connected to Office Mgr. phone not answered, no voice mail. Why is it so difficult to make an appt. with this practice? I have worked in Health Care for 30 years, and really do not understand why it is so difficult to see a physician for a Knee problem.

I have scoliosis ,had surgery when I was 15 and 16 I am now 48, My question is does Dr. Tally treat adult scoliosis? If so is he taking new patients. Thank you for addressing question Arlene
