Appointments

We must have authorization form from your employer or your Workers’ Comp adjuster BEFORE we can schedule your appointment or see you as a walk-in. If you submit an online request, our office will contact you as soon as possible. If you submit a request after-hours, our office will contact you during normal business hours. If you are an Urgent Care walk-in, our Workers’ Comp team is working as quickly as possible to receive an authorization for your visit today.

Department of Labor (DOL)

If you are an employee with the Department of Labor, please make sure you notify us. You will either need to give us your claim number for us to check eligibility on the DOL portal or submit a valid CA-16. The CA-16 will need to be completed correctly by your employer. Our office does not supply the required forms. These must come from your employer.

FMLA/Disability

If you are planning to apply for FMLA/Disability, please contact your employer's Human Resources (HR), department for assistance. FMLA/Disability forms are unique to each employer. Your company will provide you with the form(s) they require AOC to complete.
