If you have an injured employee needing an appointment, we accept submissions through our website, email, fax, or by phone. Our office will need authorization from you, the employer, or your insurance carrier, before we can schedule an appointment with your employee.
If you submit an online request, email, or fax, please expect a call from our office promptly. At this time, please have the below information readily available. This will allow us to quickly take care of you and your employee.
Employer Self-Pay Policy
If you choose to private pay for your employee’s treatment and not file claims under your Workers’ Comp insurance, please read the below information.
There will be a $750 deposit due at the time of scheduling or at the time of your employee’s appointment. Please note that this is only a deposit and treatment costs could potentially exceed $750. If office visits exceed this amount, our clinic will send bills to the billing address you provide during the intake process. If diagnostics, surgery, injections, or any other procedure/service is ordered by the provider, please note that 100% of the total deposit will be required before services are performed. These fees will not be discounted and our office will follow WC protocols and bill based off of the GA Fee Schedule.
If no further treatment is needed, and there is a remaining credit, you will be reimbursed with the remaining amount.
